Define the metadata for your survey form. You can build the form structure using the Survey Creator after saving.

URL Alias *

/survey/{tenant}/{env}/

This will be the URL path to access the form. Only letters, numbers, hyphens, and underscores.

PDF Template File

Name of the PDF template file to render survey results to.

PDF Transform Script

Name of the JavaScript file to transform JSON form results into the PDF.
